Assignment of Insurance Benefits
Foster Family Dentistry gladly accepts assignment of insurance benefits as a courtesy to our patients. Dr. Foster makes every effort to make decisions in the best interest of his patients, not the insurance companies; insurance and payment options are estimates only. We also understand that insurance plans vary and that as such there may arise situations that require you to pay for procedures out of pocket. In order to help you during these situations, we provide you with an estimate of your portion of the bill and offer a variety of flexible payment arrangements and interest free options for treatment. Payment arrangements are requested at the time of your visit.

Our office is a fully approved and an accredited user of the Visa and Mastercard Health Care Program, which will enable you to use your credit card to automatically cover the amount not paid by your insurance. You may also choose a comfortable amount to be automatically billed to your credit card on a monthly basis.
Accounts outstanding, including delinquent insurance payments, more than 60 days from the treatment date, will bear an interest at 1 1/2% or 18% per annum.
All major treatment, as well as treatment involving a laboratory procedure, will require an appropriate down payment.
